Require name/email/phone tooltip

This feature collects first name, last name, email address, and phone number during the ticket-buying process.

• Select “no” to not require attendees to fill out the first name, last name, and email address.
• Select “yes, per ticket type” to require attendees to fill out first name, last name and email address once per ticket type (i.e. if an attendee orders two tickets of the same type, s/he will complete the information once).
• Select “yes, per attendee” to require attendees to fill out first name, last name, and email for each ticket ordered (i.e., if an attendee orders two tickets of the same ticket type, s/he will complete the survey twice).

To require phone number, select "yes" under the require phone section.
